Application Process
Volunteer Member Application Packets may be obtained by completing one of the following options:

1. Download & Email The Volunteer Member Application Packet
   Fill out the form, then click the EMAIL FORM button located on the top of the application to submit
   
   
   • Volunteer Member Application Packet
     (Do not sign the application digitally as it can be signed in-person at a later date)
     
     
   • DMV- Order Your Own Record
     (Mail to: DMV Records Services, 1905 Lana Avenue, Salem, OR 97314)
     
     
2. Pick Up A Volunteer Member Application Packet
   Available for pick up at the Volunteer Services Office located at:
