Public Records Requests
Clackamas Fire District #1 maintains several different types of public records, to include: fire investigation reports, incident reports, medical records, and occupancy inspections. The District does not maintain records for building permits, or underground/aboveground storage tanks.
The processing fee is $12.00 per report. If you were directly involved in the requested incident (i.e., patient, homeowner, building owner, etc.) there is no processing fee for your request. Please contact us if you have any questions about whether or not the processing fee is applicable to you.
There is a $5.00 fee for CDs of investigative photos. Please note that these are typically only available for investigated fires. If you would like to know if there are photos available for your request, please contact us.
All checks should be made payable to Clackamas Fire District #1. We are unable to accept or process online payments at this time.
To request a copy of a fire investigation report, fill out the Public Records Request Form. Be sure to include the date and location of the fire, in order to ensure that staff are able to easily locate the records. Please note that fire investigation reports may take several weeks to complete, depending on the nature of the incident.
To request a copy of an incident report, fill out the Public Records Request Form. Be sure to include the date and location of the incident, in order to ensure that staff are able to easily locate the records. If you are looking for the history of responses to a particular location, please indicate a date range. Additionally, please indicate the types of incidents that you are interested in in the “Other Important Information” field of the form. Note: If your request yields multiple incident reports, and you would like a copy of each one, you will be charged the $12 processing fee for each incident.
To request a copy of a medical record, please fill out the Public Records Request Form and the Medical Records Release Form. For more information about medical records requests, please go here.
To request a copy of an occupancy’s inspection report, please fill out the Public Records Request Form. Unless otherwise requested, you will receive a copy of the most recent inspection form. Please do not include more than one occupancy or address on each request form. Including multiple occupancies or addresses will result in additional processing fees.
